Epilepsy

Epilepsy refers to repeated episodes of seizures. It is a chronic non-communicable disease of the brain that affects people of all ages. It is characterized by recurrent seizures that may involve a part or the entire body. Patients with seizure may lose consciousness and control of bowel or bladder function. Partial seizures (also known as focal seizures) are the most common type experienced by patients with epilepsy. Antiepileptic drugs are required to prevent the reoccurrence of seizures.

Valproate
Valproate used to be the first-line treatment option for most patients with epilepsy. However, it is found to pose a high risk of congenital malformations and neurodevelopmental disorders in pregnancy. As lifelong treatment is usually required, alternative treatment options should be considered for females.

Topiramate
Topiramate is indicated as monotherapy or adjunctive therapy in partial seizures and primary generalized tonic-clonic seizures. It is also indicated as adjunctive therapy in seizures associated with Lennox-Gastaut syndrome. Topiramate should not be used in pregnancy due to an increased risk of congenital malformations.

Levetiracetam
Levetiracetam is indicated as monotherapy or adjunctive therapy in partial seizures. It is also indicated as adjunctive therapy in myoclonic seizures and primary generalised tonic-clonic seizures. It is available as tablets, oral solution and parenteral solution. Unlike most antiepileptic drugs, Levetiracetam can be used during pregnancy if clinically needed.

Gabapentinoids
Gabapentin and Pregabalin are indicated as adjunctive therapy in partial seizures, generally reserved for patients refractory to other adjunctive therapy. They are also commonly used in neuropathic pain due to the analgesic activity of gabapentinoids. There are no adequate data from the use of Gabapentin and Pregabalin in pregnant women.
