Glaucoma
Glaucoma is one of the leading causes of blindness in Hong Kong. Due to increased intraocular pressure as a result of ageing, trauma or steroid use, patient’s field of vision become constricted, leading to progressive loss of vision. Different classes of drugs are available for the treatment of glaucoma.
Beta blockers
Beta blockers (e.g. Timolol) exert their therapeutic action by reducing the production of aqueous humor and decreasing ocular pressure. Timolol was the first beta blocker approved for topical use in the treatment of glaucoma. It continues to be the gold standard against which all new glaucoma treatments are measured. The side effects of beta blockers are minimized due to topical administration.
Prostaglandin analogues
Prostaglandin analogues (e.g. Latanoprost, Travoprost) are another first-line therapy for glaucoma. They exert their therapeutic action by increasing uveoscleral outflow and increasing the permeability of tissues in ciliary muscle. They only require once daily administration in the evening, thus are considered as a convenient choice for patients, especially those with compliance issue. Latanoprost and Travoprost/Timolol are available in a preservative-free formulation.
Carbonic anhydrase inhibitors
Carbonic anhydrase inhibitors (e.g. Brinzolamide, Dorzolamide) exert their therapeutic action by inhibiting CA-II in the ciliary body. This slows down the production of bicarbonates and reduces the secretion of aqueous humor. They are used as an adjunct with Timolol to produce additive ocular hypotensive effect. Dorzolamide/Timolol is also available in a preservative-free formulation.
Alpha-2 agonist
Brimonidine is a highly selective alpha-2 agonist. It exerts its action by reducing the formation of aqueous humor. It may be used as a monotherapy in patients who cannot tolerate topical beta blocker therapy, or as an adjunct to other classes of drugs.
Trabeculectomy
Trabeculectomy is the most effective treatment to lower intraocular pressure. However, excessive post-operative scarring can lead to surgical failure. In US, topical Mitomycin is indicated as an adjunct to ab externo glaucoma surgery to reduce the risk of surgical failure.
