MRSA Infections

Staphylococcus aureus is an opportunistic pathogen and a common cause of bacterial infections. Methicillin-resistant S. aureus (MRSA) is a strain of S. aureus with multiple drug resistance to beta-lactam antibiotics, such as penicillins, cephalosporins and carbapenems. As the treatment options against MRSA are limited, MRSA infections can be critical for individuals immunocompromised and susceptible to infections, such as children and elderly.

Polypeptide antibiotics
Vancomycin has been the first-line treatment option against MRSA. However, in the recent years, a gradual increase in vancomycin resistance is observed. For MRSA infections with vancomycin MIC ≥2 μg/mL, other antibiotics should be considered.

Daptomycin is indicated in complicated skin and skin structure infections caused by susceptible Gram positive bacteria, including MRSA. It is also indicated for bacteremia caused by S. aureus, including MRSA. It is not effective in the treatment of pneumonia.

Protein synthesis inhibitors
Linezolid is indicated in pneumonia and complicated skin and soft tissue infections caused by susceptible Gram positive bacteria, including MRSA and vancomycin-resistant enterococci. Complete blood counts should be monitored weekly during linezolid therapy. As linezolid is a monoamine oxidase inhibitor, it must not be used together with drugs used in depression and hypotension. Food high in tyramine such as cheese, soy sauce and beer should be avoided.

Clindamycin is primarily used in anaerobic infections. It is also useful in skin and soft tissue infections caused by MRSA, given that the clindamycin resistance is low. Unlike Linezolid, Clindamycin has no activity against enterococci.
