Prostate Cancer
In HK, prostate cancer has been the third most common cancer in male since 2017. Prostate is a sex gland located between the male urinary tract. Prostate cancer may lead to symptoms such as painful urination, fatigue and loss of appetite. For advanced cancer, treatments include castration, antiandrogens and chemotherapy.
Castration
Castration leads to a significant decrease in male hormone production, causing the prostate cells to shrink or stop growing. Surgical castration is an operational procedure of removing both testes while medical castration involves the injection of medication, e.g. LHRH analogue. During initial treatment of LHRH analogue, patients might experience bone pain or bladder obstruction. To avoid these undesirable effects, oral medication can be used along with LHRH analogue.
Antiandrogens
Antiandrogens are the most commonly prescribed oral medication for prostate cancer. Bicalutamide is indicated for advanced prostate cancer in combination with castration. It is well-tolerated with a lower incidence of diarrhea than Flutamide. It is available in once-a-day dosing. Abiraterone Acetate is indicated for metastatic castration resistant prostate cancer. Unlike Enzalutamide, Abiraterone Acetate is not associated with seizures or other CNS side effects.
Chemotherapy
Patients may progress despite receiving castration and oral medication and this state is classified as castration-resistant prostate cancer. Docetaxel may offer survival benefit to those patients. Docetaxel infusion is given in combination with daily prednisolone as a standard initial chemotherapy regimen.
Males with family members diagnosed with prostate cancer before age 65 are encouraged to seek advice from healthcare professionals regarding the need for screening for prostate cancer.
